The nurse or therapist will answer all questions and teach simple skills for providing care and comfort for the patient. This may include:

• Reviewing the patient’s medications
• Creating a medication calendar
• Demonstrating skills like skin care, safe transfers, use of assistive devices, assistance with eating, changing bandages, establishing an exercise program, and additional instruction to help family members assist the patient
• Advising on what to expect next, how to recognize signs of problems and what to do if a problem occurs, and how to avoid common pitfalls

• A registered nurse or therapist will come to the patient’s home to assess their needs, explain the recommended services, and review the doctor’s recommended plan of care.
• Once the patient and/or HCP consent to homecare services the nurse or therapist will proceed with a complete:
  • physical assessment
  • obtain a medical history
  • discuss home care services in detail
  • develop a tailored plan of care that the patient, family and clinical team agree with
• The nurse or therapist will then contact the patient’s physician to review the developed plan and receive the doctor’s approval to implement the plan of care.
• Upon approval from the patient’s physician, the patient and nurse or therapist will set the plan of action and recovery goals.

It’s as simple as picking up the phone and speaking to one of our nurses. The nurse will listen to the needs of the patient and make a recommendation you can take to your doctor. In most cases, we can come to the patient’s home the same day or next day to assess the situation and begin providing the appropriate services.

Home Care is for anyone who:

• has been released from a hospital, rehabilitation center or nursing home.
• has recently had surgery or experienced the onset of illness.

The Lawrence Home Care nurse or therapist will assist the patient in getting services as part of the patient’s discharge plan.
