Q. When do you need home care?

A. Home care is for anyone who has been released from a hospital, rehabilitation center or nursing home; has recently had surgery; or has experienced the onset of illness.

Q. What are my home care options?

A. You can receive care from a certified home care agency; a licensed home care agency; or you can hire privately. Insurance coverage, especially Medicare and Medicaid, is based on meeting specific qualifications.

Certified:

• provides comprehensive services delivered by registered nurses, licensed physical therapists, and NYS certified home health aides.
• covered by Medicare and Medicaid if you qualify, and by most private health care insurance providers and HMO.

Licensed:

• Private duty services that may include home health aides and homemakers.
• Not covered under Medicare and most private health care insurance providers

Q. How do I select the best home care provider for me?

A. Select someone sensitive to your situation who will assess your needs, help you set goals and recommend a plan of action that includes all the resources you require. Choose a reputable agency with an experienced and caring professional staff who can provide care 24 hours a day, every day of the year. The agency should be knowledgeable about other community resources, and be able to arrange their services for you.

Q. How do I arrange for home health care services? How long will it take?

A. It's as simple as picking up your phone and speaking to one of our Registered Nurses. The nurse will listen to your needs and make a recommendation with approval from your physician. We can come to your home the same day to assess your situation and begin providing the appropriate services.

Q. What will happen during the first visit?

A. During the first visit, we follow these steps:

• A registered nurse will come to your home, assess your needs, explain the recommended services and ask for your agreement.
• If you agree with the recommendations, the nurse will proceed with a complete physical assessment, obtain a medical history and discuss your home care services in detail.
• Next, the nurse will contact your physician to review our home care service recommendations and receive your doctor's approval.
• Upon approval, you and your nurse will set your plan of action and recovery goal.

Q. How will the nurse help the family members?

A. The nurse will answer all questions and teach simple skills for providing care and comfort.
This may include:

• Reviewing the patient's medications
• Creating a medication calendar
• Demonstrating skills like: assistance with eating; changing bandages; and other instruction to help family members assist the patient
• Advising on what to expect next and how to avoid common pitfalls

Q. What if I need services that Lawrence Home care does not provide?

A. Your Lawrence Home Care Registered Nurse will make arrangements to get them.
